چکیده
OBJECTIVE To identify the causes of delayed extubation in patients planned for fast tract extubation during cardiac surgery. METHODS A prospective observational study was conducted at cardiothoracic unit of Aga Khan University Hospital for the period of one year. All elective coronary artery bypass graft (CABG) surgery patients, between the ages of 35-75 years with LVEF > or = 40 percent were included. Patients with Intra aortic balloon pump, chronic renal failure, respiratory compromise and requiring high ionotropic support were excluded from this audit. A performa was designed and later filled by the primary investigator. Patient demographics and various reasons for delayed extubation were noted in this proforma. RESULTS Total 614 patients underwent CABG surgery and 388 (63.19%) patients were planned for fast track extubation. A total of 196 (49.5%) patients could be extubated within six hours of arrival in the cardiac ICU. Common reasons for delayed extubation included deep sedation in 80 (46.5%), confusion 44 (25%), excessive bleeding in 20 (11.3%) and high inotropic support in 10 (5.68%). CONCLUSION Major contributing factors for delayed extubation were identified by this audit. These factors need to be targeted accordingly by modifications in intra operative management.

Fast Track Extubation In Adult Patients On Pump Open Heart Surgery At A Tertiary Care Hospital.
Background Fast-track cardiac surgery programs have been established as the standard of cardiac surgical care. Studies have shown that early extubation in elective cardiac surgery patients, including coronary and non-coronary open-heart surgery patients does not increase perioperative morbidity and mortality. The objective of this observational study was to determine the success and failure pro...
متن کاملEarly extubation in adult and pediatric open heart surgery; an experience from a tertiary care hospital of a developing country
Numerous remarkable advances have been made in the perioperative care of both adults and children undergoing cardiac surgery. Improvements in the technology of CPB, advances in the techniques of surgery, a better understanding of the pathophysiology of the postoperative period, and refinements in anesthetic and ICU care have led to changes in the perioperative management of these patients. Thes...
متن کاملRetrospective Analysis of Fast-Track Extubation Protocol for Cardiac Surgical Population in Tertiary Care Hospital: A 2 Years’ Experience
Background: Fast-track cardiac anesthesia aims to reduce the extubation time after cardiac surgery. Reduction in extubation time results in massive cost saving in terms of reduction in intensive care unit stay. In this era of health-care cost escalation, this can effectively reduce the burden on health-care system without compromising patient outcome. We here present a retrospective analysis of...
متن کاملThe effect of dexamethasone on the reduction of airway edema and the success of extubation in patients with head and neck surgery admitted to the intensive care unit
Background: The decision to extubate the patient is crucial and challenging. The use of corticosteroids to reduce airway edema and extubation failure in intensive care patients is debatable. Methods: In an observational prospective cohort study from April 2021 to July 2021, 110 patients undergoing head and neck surgery at Imam Khomeini Hospital's special wards were enrolled. Inclusion criteria...
متن کاملA comparison of fentanyl, sufentanil, and remifentanil for fast-track cardiac anesthesia.
UNLABELLED Cardiac surgery is estimated to cost $27 billion annually in the United States. In an attempt to decrease the costs of cardiac surgery, fast-track programs have become popular. The purpose of this study was to compare the effects of three different opioid techniques for cardiac surgery on postoperative pain, time to extubation, time to intensive care unit discharge, time to hospital ...
